We recently released the Electronic Medical Questionnaire, our online version of the Full Underwriting Application Supplement. 

Agencies and producers who have access to eApp can request the questionnaire for their clients. In the online questionnaire, clients report their personal, employment, medical and other information. And they can do it at a time and place that’s convenient for them.

The Electronic Medical Questionnaire is not available in California, Florida, Montana, New York, North Dakota, South Carolina, South Dakota and Wyoming.

The Standard has opened access to its Full Underwriting Application Supplement, known in our eApp system as the Electronic Medical Questionnaire.

The Electronic Medical Questionnaire is available for your customers residing in most states excluding California, Florida, Montana, New York, North Dakota, South Carolina, South Dakota and Wyoming. 

We process state appointments for producers at the time of application for all states except Pennsylvania. Starting April 14, the state of Washington will require that producers are licensed and appointed with The Standard before signing an insurance application for a Washington resident.
