EMQ Available in New York

The Standard’s Electronic Medical Questionnaire, or EMQ, our online version of the Full Underwriting Application Supplement, is now available in New York. EMQ is now available in all states except South Carolina.

Applicants can use the EMQ to report their personal, employment, medical and other information. It’s available through our secure online eApp platform, for applicants to complete at any time and place convenient for them.

What You Need to Know

To use the EMQ, you’ll need to be registered for The Standard’s eApp. If you already have access to eApp, you’re all set!

Agencies can start the EMQ on behalf of producers. You need only some basic client information to get started. In three simple steps, your agency can send the questionnaire to the client for completion and electronic signature.

For agencies:

The Starting Guide for Electronic Medical Questionnaire — This document gives an overview that helps producers prepare clients for reporting personal, employment and medical information.

The Producer Launch flyer — This flyer walks producers through the steps on how to set up and send the questionnaire link to their client.

For clients:
Getting Started Interactive Brochure — This interactive brochure helps prepare clients to use the EMQ. It includes an overview of the questionnaire, estimated time to complete, how to sign the questionnaire and how to spot incomplete responses in the document.
