New Business Entity Form Required

Starting July 1, 2023, all business-owned policies will need the Business Entity Resolution Certification form 22939. The form can also apply to policies owned by individuals who wish to have a business or charity listed as a beneficiary.

Unlike the Power of Attorney affidavit, you’re required to submit this new form only once. We’ll only require a new resolution certification form if authorization information changes, or if there is a new officer or agent. Go to the Find Forms & Materials button on the distributor page to get the Business Entity Resolution Certification form.

What does this form do?

  • Provides us with information to improve the legal review process.
  • Allows us to accurately track owner information and signing authorities.
  • Reduces the need for extra documentation from the business.
