Streamlined Applications for Platinum Advantage

Employees sign the new base Platinum Advantage application only once. Social Security numbers aren’t on the application. And producers sign only one Producer Information Report per case. No other signatures needed.

You may have noticed the new Platinum Advantage application. We’ve made it easier to complete.

What’s new:

The base application is only two pages. And the employee signs the application just once. You may need an additional form in some states or if the employer chose the Catastrophic Disability Benefit Rider.

Only one producer signature is required for the whole case. Producers must sign the Producer Information Report. That’s it. If you have a new addition to a case previously on Protector Platinum, you will need to sign a new Platinum Advantage PIR for the case.

No Social Security numbers on the application. We’re now asking for Social Security numbers on the census. This keeps that personal information off the applications.
