Step 1: Complete Your Claim Forms
Start by downloading a claim packet for your state:
Make sure to complete all three sections:
- Insured's Statement — Serves as a written notification of your claim for benefits.
- Authorization to Obtain and Release Information — Allows us to request medical records and other information related to your claim.
- Attending Physician's Statement — Completed by your doctor with details about your condition.
Optional
You can authorize a person to communicate with The Standard about your claim:
- Authorization to Release Information (All states except New York)
- Authorization to Release Information (New York only)
Step 2: Submit Your Claim
Send your completed forms by mail or fax using the contact information in your claim packet.
Step 3: We'll Contact You
We’ll contact you once we receive your claim and if we need any additional information.
We strive to inform you of the claim decision within 45 days.
Common Questions
Start your claim as soon as you think you may be disabled. Waiting more than 30 days may delay your benefits.
Please refer to your policy requirements for additional information about when to file a claim. The later a claim is filed, the more difficult it becomes to obtain necessary information to evaluate your eligibility for benefits.
If you have both an IDI policy and LTD coverage with The Standard, we’ll coordinate your claims and assign one claims specialist to support you.
If you do not have an IDI policy and want to file for your employer's LTD benefits, please see the filing instructions for workplace benefits.
If approved, your first payment is typically issued within 3 – 5 business days after your waiting period is met. Benefits are typically paid monthly.
It depends on how your policy was paid. Benefits are usually tax-free if you paid the premiums yourself. Check with a financial advisor for details.
Contact your claims specialist before returning to work so we can obtain the necessary details about your plans. This will assist us in determining whether additional information is needed to evaluate your claim upon your return to work.
