Filing With The Standard
If your policy is with Standard Insurance Company or The Standard Life Insurance Company of New York, you can log in to your standard.com account for the following types of claims:
- Disability
- Accident

- Hospital Indemnity
Filing With a Former Anthem Life Policy
If your policy was issued by Anthem Life Insurance Company, Anthem Life & Disability Insurance Company or Greater Georgia Life Insurance Company, please go to the Claims Service Center to file claims.
Filing With California Teacher's Association
If you receive benefits through CTA, go to the CTA Member Service site and view the instructions for How to File a Disability Claim.
