You may complete and submit the applicable PFML or PFL claim forms by mail or fax.
Online (Maine Employees Only)
If you work in the state of Maine, you can log in to your account to start a claim. Support for other states is coming soon.
If you don’t have an online account, you can create one using your employer’s policy or plan number.
New York Employees
To file a New York Paid Family Leave claim, contact the Absence Management Service Center using the phone number provided by your employer.
