Claims Payment Process

Carriers are required to provide an explanation of the process by which they will pay claims submitted by participating providers and the contact information for reviewing claims that have been denied. Please view our claims FAQ for additional information.

Iowa

Non-Covered Services Decision

The Iowa Supreme Court has determined that Iowa law does not allow dental insurers to set maximum fees on services that the insurer does not cover or reimburse. As your insurer or administrator, we will have no involvement in setting the fee for such services, and any questions or concerns you may have about such fees should be directed to your dentist.

Contracted Healthcare Benefit Managers

Carriers are required to post information about each health care benefit manager that has a contract with them. View the health care benefit managers that are contracted for dental and vision services.

West Virginia

PPO Network Access Plan

Carriers are required to make available on their website an access plan illustrating each network offered in West Virginia.
