Application Submission

Applications submitted must be for the state in which the GME program is located or in the state where the applicant will reside and practice medicine. The applicant must have a mailing address, other than a P.O. box, in that state. 

When submitting an application to The Standard, please include the following documents:

  • A completed application
  • An illustration
  • A completed Producer Information Report (Please indicate the requested effective date and all producers and commission splits on the Producer Information Report.)

Ensure the Application Is Complete

Please review applications for accuracy and completeness to minimize delays. All applications delivered electronically to The Standard's home office must be sent in a secure manner using encryption. If delivering electronically, please ensure that the applications are legible.

Common Application Errors

To avoid common errors, use The Standard’s eApp. System checks ensure all applications submitted through eApp are complete. As a result, eApps are processed more quickly than traditional paper applications. Please reference the Getting Started Guide or contact your account manager for training.

  • Applicant has not disclosed other individual disability insurance. Adding this information later may result in a change to the monthly benefit amount and will require a signature at time of policy delivery.
  • Applicant has not initialed changes to the application. Changes made after the application has been submitted may require a policy amendment.
  • The date of birth is incorrect. Changes affect the premium rate unless corrections are for the day of birth only.
  • The producer signature is dated before the applicant's signature. The Standard will require a new signature page. The producer must sign at the same time as or after the applicant signs.
  • The applicant or producer does not sign or date the application. In this case, The Standard will require a new signature page.
  • Please ensure that non-U.S. citizen applicants provide complete answers to all visa questions on the Platinum Advantage application.
