Applications submitted must be for the state in which the GME program is located or in the state where the applicant will reside and practice medicine. The applicant must have a mailing address, other than a P.O. box, in that state.
Applications must be submitted to The Standard through the eApp system. The eApp system will ensure the correct application and forms are presented to each client. In addition to the application, an illustration is required. Please reference the Getting Started Guide or contact your associate account manager for training.
Common Application Errors
Applicant has not disclosed other individual disability insurance. Adding this information later may result in a change to the monthly benefit amount and will require a signature at time of policy delivery.
Ensure that non-U.S. citizen applicants provide complete answers to all visa questions on the application.
