All late applications and requests for coverage increases are subject to medical underwriting approval. If medical underwriting approval is required, complete and submit your medical history online or complete and submit a hardcopy Medical History Statement if preferred. Medical underwriting approval is also required for reinstatement of terminated coverage and for members eligible but not insured under the prior life insurance plan.
