Life and AD&D Insurance
Basic Life & AD&D (BLADD)
Your employer provides you with a basic level of coverage. Will that be enough for you? Think about your ongoing expenses and how much income is needed in order for your loved ones to maintain their lifestyle in the event of your death. Basic Accidental Death and Dismemberment (AD&D) insurance may provide an additional amount due to dismemberment as a result of an accident.
Included With Your Coverage
Your Basic Life coverage includes:
- Travel Assistance: Offers aid before and during your trip, including visa information, health inoculation recommendations, credit card and passport replacement and connection to medical care providers
- Life Services Toolkit: Provides resources for financial planning, estate planning assistance, health and wellness, identity theft prevention and funeral arrangements
Additional Life & AD&D (ALADD)
If Basic Life insurance doesn't cover your existing financial obligation, Additional Life insurance may help to bridge that gap. Plan now to help your family cover expenses like tuition, childcare, housing and daily living expenses.
- For you - As an active employee, you can elect up to 5x your annual earnings, not to exceed $500,000. The Guarantee Issue amount is $350,000. You may also elect AD&D coverage, which no longer needs to match your Life benefit. If insured for Plan 2 Life insurance, you may apply for Plan 2 AD&D insurance. You may apply for options of 1 through 5x your annual earnings, rounded to the next multiple of $1,000, if not already a multiple of $1,000. The maximum amount is $500,000.
- For your spouse - Coverage is available in increments of $5,000 up to $250,000, not to exceed 100% of your Additional Life insurance amount. The Guarantee Issue amount is $50,000. If your Spouse is insured for Dependents Life insurance, you may also apply for Dependents AD&D insurance for your Spouse in increments of $5,000 up to $250,000. AD&D Insurance for your Spouse may not exceed 100% of your Additional AD&D Insurance.
- For your child(ren) – The Child Life insurance amount is $10,000, not to exceed 100% of your Basic Life amount. Coverage is from live birth through age 30. EOI is never required for Child Life insurance. If your Child is insured for Dependents Life insurance, you may apply for Dependents AD&D insurance for your Child. The amount of Dependents AD&D insurance benefit for your Child is equal to the amount of Dependents Life insurance benefit for your Child.

You may elect a basic amount of Life insurance if you are an eligible retired employee. You must apply and agree to pay premium before your coverage becomes effective.

Short Term Disability (STD) Insurance
STD coverage replaces 66 2/3% of your weekly income when you can’t work because of a qualifying disability, including injury, physical disease, pregnancy or mental disorder. You can choose from two different elimination periods:
- Plan A: 30 day elimination period and 22 week maximum duration
- Plan B: 15 day elimination period and 24 week maximum duration

Examples of how Short Term Disability insurance can help protect your income:
Pregnancy
- Emma gives birth.
- She missed six weeks of work to recover from her pregnancy, which also allowed her to be with her baby.
- Her paychecks stop, but there are still expenses related to having a newborn.
- Short Term Disability coverage pays part of her wages during this time, which helps Emma take care of her new family member.
Back Surgery
- Robert has back surgery.
- He can’t work for several months.
- His income stops but his mortgage, child care and other bills keep coming.
- Short Term Disability coverage pays part of his earnings while he’s disabled, helping him keep his finances on track.

Long Term Disability (LTD) Insurance
LTD coverage replaces 66 2/3% of your monthly income for qualifying disabilities, whether you are out for a few months or years. LTD can also help you get back to work when you’re ready.
180-day elimination period and a benefit duration to age 67 or other duration based on age at time of disability.

Examples of how Long Term Disability insurance can help protect your income:
Cancer
- Bella had a heart condition, then was diagnosed with cancer.
- Chemotherapy left her too drained to work her construction job — and facing extra expenses such as co-pays and in-home care.
- Long Term Disability coverage kept part of her pay coming in — and her retirement savings intact.
Car Accident
- A severe car accident left Carl with back injuries and unable to work for several months.
- His Long Term Disability plan replaced part of his paycheck. That helped him pay for his rent, groceries, kids’ birthday presents and other expenses.
- Carl’s insurance also paid for an adjustable desk and chair that helped him work part-time while recovering. He received partial LTD benefits until he could work full-time.

The School Board of Volusia County, Florida makes it easy to increase or apply for coverage. The Decision Support Tool is an interactive tool that includes plan specific benefit summaries, short educational videos and interactive calculators to assist you in making decisions about your supplemental coverage and lets you take action linking directly to your online enrollment system.
If coverage becomes effective, and you become insured, you may request a group insurance certificate containing a detailed description of the insurance coverage including the definitions, exclusions, limitations, reductions and terminating events. The controlling provisions will be in the group policy.

You can create a new account or log in to an existing account to file a claim.
You may also complete and mail your disability claim forms.
