Employee Resources
Long Term Disability Insurance
Group Long Term Disability helps protect your employee's income if they are unable to work for an extended period of time due to a covered illness or injury. This benefit is designed to replace a portion of their income to help meet monthly expenses when they may need it most.
Coverage Amount
Two Coverage Levels Designed to Fit Your Employees' Needs
Eligible Excluded employees can choose one of the following options:
- Option A: Replace up to 65% of the first $15,385 of their monthly base salary reduced by deductible income
- Option B: Replace up to 55% of the first $18,182 of their monthly base salary reduced by deductible income
Maximum Benefit for Both Options: $10,000 before reduction of deductible income
Minimum Benefit for Both Options: $100 or 15% of their benefit before reduction by deductible income, whichever is greater

Benefit Waiting Period
Benefits become payable after a six-month waiting period during which the employee is continuously disabled. Employees are responsible for making monthly premium payments during the benefit waiting period.
Maximum Benefit Period
Determined by the employee's age at the time of disability, benefits may continue until age 65, the Social Security Normal Retirement Age, or under the plan’s age-graded schedule, whichever is greatest. Refer employees to the Certificate of Insurance for more details.
Benefit Premium
The premium payment is determined by the benefit option the employee elects. Payments are conveniently managed by automatic payroll deduction.

- Survivors Benefit equal to six times the unreduced Group Long Term Disability benefit may be payable to the employee's survivors if they die while benefits are payable and has been continuously disabled for at least 180 days. Any Survivors Benefit payable will be first applied to any overpayment of their claim due to The Standard.
- Dependent Education Benefit may be payable if the employee has a dependent under the age of 22 attending an accredited post-secondary school on a full-time basis while they are disabled. An additional $150 (increasing to $300 in 2026) per month for up to 48 months for each child is available.
- Family Care Expenses Benefit adjusts any calculated work earnings offset if the employee is participating in an approved rehabilitation plan and incur child or elder expenses.
- Conversion allows employees the option to purchase Long Term Disability conversion insurance after the termination of their insurance if they meet the requirements defined by the group policy.
Employees may be eligible to continue their LTD coverage in certain circumstances as described below.
24-month Direct Pay for LTD Insurance
Under the provisions of the Group Long Term Disability coverage provided by Standard Insurance Company, employees may have the right to continue their coverage for up to 24 months if they cease to be eligible due to a classification change. If they wish to continue their coverage, they must make the election to continue their insurance within 60 days following the date they cease to be a Member. If they do not apply to continue their insurance during that 60-day period, their insurance will be deemed to have ended on the date they ceased to be a Member. If they apply to continue their insurance during that 60-day period, their coverage will be retroactive to the date they ceased to be a Member.
Premium is due on or before the first day of each calendar month, and is paid directly to us at our home office. If they do not pay a premium before its due date, they may pay the premium during the following 60-day grace period. If they do not pay the premium during the 60-day grace period, their insurance will terminate automatically at the end of the grace period.

Conversion of LTD Insurance
Employees may be eligible to convert their coverage to an individual insurance policy if all the following conditions are met at the time their insurance ends:
- Their insurance ends for a reason other than:
- Termination or amendment of the group policy;
- Failure to make a required premium contribution; or
- Retirement.
- They were continuously insured under State of California's group Group Long Term Disability plan for at least one year as of the date their insurance ended;
- They are not disabled on the date their insurance ends;
- They are a citizen or resident of the United States or Canada;
- They apply in writing and pay the first premium for the individual disability income policy within the 31 days after their insurance ends.
The effective date of the individual disability income policy will be the day after their insurance ended under the Group LTD policy.
Please note that Long Term Disability Conversion Insurance is not a continuation of their insurance under the group Group Long Term Disability policy which State of California has with The Standard. It is a separate and distinct policy which may provide different benefits, exclusions, limitations and reductions than those provided under the group policy. Accident Insurance
The expenses of an accident can add up fast, especially when it keeps employees from working. Group Accident Insurance pays benefits for covered injuries and treatments to employees and their loved ones, not to providers. Employees will get a lump sum to spend on whatever they need, from deductibles and copays to groceries and alternative treatments.
If employees are enrolled or enroll in the Group LTD policy 643146 through the State of California, they are automatically enrolled in Group Accident Insurance at no cost if they are an eligible Excluded employee.
If employees are not enrolled in the Group LTD policy 643146 through the State of California, they’ll have access to Group Accident Insurance at affordable group rates.
Coverage Amount
Group Accident Insurance includes more than 70 benefits for covered injuries and treatment. Refer employees to the Employee Benefits Guide for more information.

- Health Maintenance Screening Benefit pays a $100 benefit once per calendar year when employees and their spouses or domestic partners go to the doctor for a covered wellness screening, which may include a novel infectious disease test (including COVID-19) or a mammogram.
This coverage is portable. That means that employees may be able to continue their coverage through direct bill if their employment ends, the group policy terminates or their insurance ends because they no longer meet the eligibility requirements. Direct the employees to begin the application process:

Critical Illness Insurance
Health insurance can cover many of the medical expenses related to a serious illness, but it doesn't cover everything. When it comes to co-pays, deductibles, loss of income, child care and even travel expenses, Group Critical Illness Insurance can help fill that financial gap with a safety net of coverage for employees and their loved ones.
If employees are enrolled or enroll in the Group LTD policy 643146 through the State of California, they are automatically enrolled in Group Critical Illness Insurance at no cost if they are an eligible Excluded employee.
If employees are not enrolled in the Group LTD policy 643146 through the State of California, they’ll have access to Group Critical Illness Insurance at affordable group rates.
Employees and their spouses or domestic partners may continue coverage past age 64. However, they cannot apply for new coverage past age 64. Employees and spouses or domestic partners must have major medical or other minimum essential insurance that provides medical, hospital, and surgical coverage to be eligible for Group Critical Illness Insurance.

- Health Maintenance Screening Benefit pays a $100 benefit once per calendar year when employees and their covered dependents go to the doctor for covered wellness screenings, which may include a novel infectious disease test (including COVID-19) or a mammogram.
- Child(ren) are automatically covered at 50% of the amount elected for employees for the same critical illnesses that employees are. Child(ren) are also covered for 21 additional childhood diseases, including cystic fibrosis, Down syndrome, muscular dystrophy, spina bifida and cerebral palsy.
- Additional benefits included if employees and covered dependents are diagnosed with a covered illness again after a treatment-free period of six months. They will receive 100% of the original benefit amount. If they are diagnosed with a different and subsequent covered illness after the diagnosis of the first critical illness, they will receive an additional Critical Illness Insurance benefit.
- Additional services available through Health Advocate include access to specialists for a second opinion upon approval of a covered claim.*

Payment of benefits is subject to the terms and conditions of the group critical illness policy and insurance certificate. These plan documents are the final arbiter of coverages.
