Once you've had a chance to review the Additional Life Insurance Booklet and the Certificate of Insurance, along with the Long Term Disability Insurance Booklet and Certificate of Insurance and you're ready to enroll in coverage, you'll want to complete the Enrollment and Change Form. You may enroll during the following periods:
Initial Enrollment Period (New Hires)
You may enroll for coverage within 31 days of becoming a Member.
Family Status Change Period
You may enroll for coverage within 63 days of a Family Status Change
Annual Enrollment Period
You may enroll during Montana University System's designated annual enrollment period held each spring. Please contact your human resource representative for specific dates.
If you are required to provide evidence of good health, you will need to complete a Medical History Statement. If you have questions, please speak with your human resource representative regarding your enrollment options.
