Government of the District of Columbia provides you with the opportunity to purchase Basic Life and Basic AD&D insurance (Plan 1). Basic Life and AD&D insurance becomes effective immediately, if you are actively at work on that date. Basic Life insurance is equal to one times your Annual Earnings plus $2,000 and the amount of AD&D coverage is equal to the amount of life coverage. Basic Life has a minimum of $10,000.
Eligible employees enrolled in Basic Life and AD&D (Plan 1) may also elect any or all the following insurance options:
- Optional Life Insurance (Plan 2, Option A) - A flat $10,000 benefit
- Optional Life and AD&D Insurance (Plan 2, Option B) - A choice of one, two, three, four or five times your Annual Earnings
- Dependents Life Insurance (Option C) - Dependents coverage is available in the following amounts for your spouse and each child. The amount of Dependents Life insurance may not exceed 50 percent of the amount of your Life insurance.

In addition, you may qualify for an Extra Benefit amount. This provides an extra percentage of the amount of your Basic Life insurance benefit, from 10 percent to 100 percent depending on your age at the time of death. The Extra Benefit is payable only if you die before age 45.
In certain circumstances, you may be required to provide evidence of insurability.
