Group Long Term Disability Insurance for Excluded Employees

 

Coverage Continuation Options

As an insured Member under State of California's group Group LTD policy, you may be eligible to continue your LTD insurance in certain circumstances as described below.

24-month Direct Pay for LTD Insurance

Under the provisions of the Group Long Term Disability (LTD) insurance coverage provided by Standard Insurance Company, you may have the right to continue your coverage for up to 24 months if you cease to be eligible due to a classification change. If you wish to continue your coverage, you must make the election to continue your insurance within 60 days following the date you cease to be a Member. If you do not apply to continue your insurance during that 60-day period, your insurance will be deemed to have ended on the date you ceased to be a Member. If you apply to continue your insurance during that 60-day period, your coverage will be retroactive to the date you ceased to be a Member.

Premium is due on or before the first day of each calendar month, and is paid directly to us at our home office. If you do not pay a premium before its due date, you may pay the premium during the following 60-day grace period. If you do not pay the premium during the 60-day grace period your insurance will terminate automatically at the end of the grace period.

Conversion of LTD Insurance

You may be eligible to convert your coverage to an individual insurance policy if all the following conditions are met at the time your insurance ends:

  1. Your insurance ends for a reason other than:
    1. Termination or amendment of the group policy;
    2. Your failure to make a required premium contribution; or
    3. Your retirement.
  2. You were continuously insured under State of California's group Group LTD insurance plan for at least one year as of the date your insurance ended;
  3. You are not disabled on the date your insurance ends;
  4. You are a citizen or resident of the United States or Canada;
  5. You apply in writing and pay the first premium for the individual disability income policy within the 31 days after your insurance ends.

The effective date of the individual disability income policy will be the day after your insurance ended under the Group Long Term Disability policy.

Please note that LTD Conversion insurance is not a continuation of your insurance under the group Group LTD insurance policy which State of California has with The Standard. It is a separate and distinct policy which may provide different benefits, exclusions, limitations and reductions than those provided under your group policy.
