If you are submitting an anticipatory claim where the employee has not yet ceased work, please be aware that The Standard cannot accept claims that are submitted more than 60 days prior to the anticipated work cessation date.
Partially completed submissions cannot be saved. Please review the form prior to starting and ensure that you have all the required information available. Some information may be prefilled for your convenience but can be changed if it is incorrect. Employer, contact and tax information can be saved on your computer to ease future submissions.
Complete all required fields on the employer statement and submit the form.
You will receive either a submission confirmation or a processing error with correction instructions. If you do not receive one of these responses, please call our customer service line at 800.348.3226.
Print the Employee Claim Packet using the link on the left side of the screen. If you have multiple policies with The Standard, select the applicable policy number first to ensure that you receive the appropriate packet.
Give the Employee Statement, Authorization and Attending Physician Statement to the employee. The employee and physician are to send these forms directly to The Standard.
