Updated Platinum Advantage Application Packets

We've updated Platinum Advantage application packets with a new IDI Application Checklist and Cover Sheet as well as a new form to initiate the online credit card payment process for conditional receipt.

The updated application packets begin with the IDI Application Checklist and Cover Sheet. We designed this checklist to help producers stay on track as they guide their clients through the income protection insurance application process. Producers can use the checklist to be sure they’ve submitted all required forms, asked all the right questions, and provided their client all forms required by law. The checklist also provides compliance tips to help avoid common roadblocks that can delay the application and underwriting process.  

You'll also find the new One-Time Premium Payment for Use With Conditional Receipt form in the updated packet. This new payment form replaces the Authorization for One-Time and/or Recurring Electronic Funds Transfer (EFT) form the old packets provided. Applicants can pay for conditional receipt with a credit card or EFT using this form in all states where Platinum Advantage is approved, except New Jersey and New York. Applicants must submit the Disability Insurance Conditional Receipt form at the time of application. Please see the Conditional Receipt – Paying by Credit Card or EFT flyer for more information.
