Platinum Advantage Application Forms Updated

Sometimes we receive application forms that are hard to read because they were scanned or faxed multiple times to collect information and signatures. Most forms are legal documents, so they must meet legibility requirements. Legal forms with blurred, faded or missing verbiage or form numbers can compromise the legality and defensibility of the document.

To help improve legibility, we have changed the font in all Platinum Advantage application forms. We believe the new font will be less likely to fade or blur. Please discard any previously printed Platinum Advantage applications and forms and begin using the updated versions.

Our goal is to accept as many forms submitted as possible, and our hope is that the updates made to the forms will reduce the number of times we have to request a better copy or a new form.

How You Can Help Increase Form Legibility:

• Please try to limit the number of times a form is copied, scanned or faxed.
• Review copier, scanner and fax settings to ensure the most legible copy possible.
