New and Improved Forms and Applications

The Applications & Forms page on The Standard's website has been redesigned to provide a consistent look and feel with the rest of The Standard's website and offer a streamlined approach to help you quickly find the forms you need.

You can find the Applications & Forms link on the left at



On the Applications & Forms page, choose the state in which the applicant or policyowner lives and the type of form needed — new business, increase options or a service form — to quickly see a short list of the possible forms needed.

The forms displayed can be further narrowed down by choosing the product under “Filter by Product” found in the right column.



The applications and forms displayed will be separated into two groups: required forms and forms needed for specific situations or conditions.
