Claim Process for Policyowners

Do your producers have questions about the claim process? After speaking with a claim specialist by calling 800.628.9797, the policyowner will be sent a claim packet. The policyowner will then complete and submit the claim packet, which includes the Insured's Statement, Attending Physician's Statement and Authorization to Obtain Information form.

Sometimes producers want to help by assisting with the claim packet and inquiring about a claim on behalf of a client. However, it’s important that the policyowner complete the packet and make any inquiries themselves, for two reasons:

  1. The policyowner has all the necessary information to fully complete the packet and answer follow-up questions.
  2. All information about the client and their claim is treated with the utmost confidentiality.

We understand that filing a disability claim may come during one of the most challenging times in a policyowners’s life, and that producers wish to help their clients as much as possible. Producers can feel confident knowing that our goal is to deliver exceptional service, providing expert and friendly service we wish for ourselves and our families.

Producers can help their clients by encouraging them to file a claim as soon as they believe they may be disabled. It's better to start the claim process right away, even if they ultimately recover sooner than expected and don’t qualify for benefits. A complete claim investigation will be conducted once a claim notice, forms and any requested additional information (if applicable) are received. Therefore, if a client waits until late in the benefit waiting period to contact the claims department, a decision regarding payment of benefits may not be reached until after the waiting period ends.
