Effective June 1, 2009, the Guarantee Issue Underwriting Request, Form 8041, will be revised so it automatically captures case data needed to underwrite, illustrate and manage GI cases efficiently. I believe as a result, that you will see leaner processing time for the GI cases you submit to us.

The new GI Underwriting Request Form will be available at www.standard.com/di via Find Forms/Service Forms. It has fewer required fields than the prior form, allowing you to provide only basic information before you can submit it to The Standard. However, we do encourage you to provide as much information as possible so we can provide the most competitive offer.
Once you submit the completed form, the saved or printed summary of the form will include a coverage summary that provides a side-by-side comparison of the requested plan design(s).
To obtain copies of the GI Underwriting Request Form you either enter your email address on the first page of the form, as indicated below,
or you can click the Print Request button on the last page of the submission.
Click Submit Request to complete the form submission. Printing or saving the document does not submit the form.
Sending confidential information
  In order to send confidential information like a census related to a GI Underwriting Request, please email GIsales@standard.com to request a secure drop. Within four business hours, the GI Team will reply to your email and include instructions on how to safely upload the information.
