The easiest way to apply for coverage is by filling out an online form. The applicant automatically receives a confirmation email upon submission of the form. |

The Medical History Statement form is required if applying outside of a special enrollment opportunity. This form can be printed and also submitted online if the Adobe Acrobat Reader browser plug-in is enabled. |

The Beneficiary Designation Form is an important piece that many members forget to fill out when they enroll in Disability and/or Life Insurance coverage. This form can be printed as well as filled out and submitted online if the Adobe Acrobat Reader browser plug-in is enabled. |

This form is for members who need to update their information on file with The Standard such as a new name, address, salary or to request a reinstatement of coverage. |
