Try Out the New Electronic Medical Questionnaire

Your agency and producers can start using the Electronic Medical Questionnaire today.

In addition to starting the Electronic Medical Questionnaire with your clients, we encourage you to explore our new system and see how it works over the next month. Trying out the system will allow you and your team to:

• Walk through starting a questionnaire for your clients
• Take a look at the client’s view of the questionnaire (if you send it to yourself)
• See the information requested in the questionnaire
• Check your All Activities tab to see how a completed questionnaire shows up

Test Guidelines to Remember

When requesting a new Electronic Medical Questionnaire, indicate TEST in the naming of the application. This visual aid shows where you'll rename the questionnaire, under Create Activity, in the system.
The system testing period runs from April 5 to May 5.
