Obsolete Applications and Forms Not Accepted After Feb. 28

In January, The Standard implemented several updated application, authorization and service forms due to required changes. The changes affected forms in the Authorizations and Disclosures, Application for Policy Increase and Benefit Renewal, and Application for Reinstatement or Policy Change packets for all states.

For California, we also made required changes to applications for New Business, Increase Options, and Policy Change and Reinstatement. As part of our changes in California, we’ve added an application endorsement to the application forms. This is a required document that updates the fraud notice in the application. It must be presented to the applicant at time of application completion and submitted to The Standard with the other application pages.

All packets or documents downloaded before January 2023 should be deleted as they are now obsolete. Any obsolete forms received after Tuesday Feb. 28 will not be accepted.* Outdated forms received after that date will mean that you and your clients will need to sign and submit new forms.

Please delete any previously downloaded packets and documents to prevent processing delays or rejections. 

If you are using eApp, no additional actions are needed. Only up-to-date forms were available since the required changes were implemented. All updated documents and packets are available in our Application and Forms directory.
