Electronic Medical Questionnaire Now Available

Starting today, you have access to The Standard’s Electronic Medical Questionnaire, the Full Underwriting Application Supplement. The questionnaire is available through our eApp system, a convenient, secure online platform.

Agencies and producers who have access to eApp can request the questionnaire for their clients. In the online questionnaire, clients report their personal, employment, medical and other information. And they can do it at a time and place that’s convenient for them.

The Electronic Medical Questionnaire is currently available for clients residing in most states. It is not available in California, Florida, Montana, New York, North Dakota, South Carolina, South Dakota and Wyoming.

What You Need to Know

To use the Electronic Medical Questionnaire, you need to have access to the eApp system. If you or your producers already have access to eApp, you’re all set! If you’d like to have agency staff or producers provisioned for eApp, contact your sales consultant.

We encourage you to review important information online for use of the questionnaire. You’ll find guidance for the Electronic Medical Questionnaire and Producer Information Report. You can review this information and share with your team.

Agencies will need to review PIRs for all paper applications and eApplications to determine if an Electronic Medical Questionnaire was selected. To initiate the Electronic Medical Questionnaire on behalf of a producer, you can use the agency flyer, below, for guidance.
