Conversion Guidelines for Long Term Disability Insurance

When your Long Term Disability (LTD) insurance ends, you may have the right to convert that insurance to an individual disability income policy. You'll find information about your LTD coverage continuation options below.

As an insured Member under the Schwan's Shared Services group LTD policy you have the right to convert your LTD coverage if all the following conditions are met at the time your insurance ends:

1. Your insurance ends for a reason other than:
   1. Termination or amendment of the Group Policy;
   2. Your failure to make a required premium contribution; or
   3. Your retirement.
2. You were continuously insured under the Schwan's Shared Services, LLC group LTD insurance plan for at least 12 months as of the date your insurance ends;
3. You are not disabled on the date your insurance ends;
4. You are a citizen or resident of the United States or Canada;
5. You are under age 60; and
6. You apply in writing and pay the first premium for the individual disability income policy within the 60-day period after termination of insurance.

The effective date of the individual disability income policy will be the day after the last day of insurance under the Group Long Term Disability policy.

Please note that LTD Conversion insurance is not a continuation of your insurance under the group LTD insurance policy Schwan's has with The Standard. It is a separate and distinct policy which may provide different benefits, exclusions, limitations and reductions than those provided under your group policy.
