As the employer, you are responsible for submitting the Employer's Statement of the Life, STD and LTD claim forms to the Printers' Disability Trust (PDT) for review and submission to The Standard.
For all coverages, including Life, STD and LTD, you may complete a paper form (see below) and submit it to the Trust by fax or mail. To access the paper form, please use the link for the appropriate coverage. If you have questions or require assistance, call PDT at 1.800.342.5799.
At claim time, please provide the employee with the appropriate claim forms, which are available in the Forms section at the top of this web page.
