New Hires
New hires are eligible for Additional Life coverage up to three times annual earnings without evidence of insurability.
Late Enrollees
Evidence of insurability is required for late applications for Life and Dependent Life insurance. Employees and dependents are considered late enrollees if they meet the plan rules for eligibility but do not enroll within 31 days of becoming eligible.
- For coverage requests in excess of three times annual earnings the member will be required to provide evidence of insurability. You may access the Medical History Statement here or online at the New Mexico General Services Department website, forms section. Please complete all items, date, and sign as instructed.

Please allow six to eight weeks for full process time of your Medical History Statement. A physical exam and/or physician statement may be required. The Standard will notify you of any requests and decisions via the mailing address or phone number you provide.
Group Representatives
- Evidence of insurability decisions/status can be viewed by logging on to The Standard's online Evidence Reporting system (http://adminease.standard.com/).
- When submitting the Medical History Statement form to The Standard for review, the Group Representative should include a fax cover sheet with the representative's contact information and the policy number (645553). This will allow The Standard to contact the appropriate Group Representative with any questions.
- Once approval has been received, Group Representatives should then enter the member election into PeopleSoft. Local Public Bodies should make a screen print of the approval from the Evidence Reporting system and attach it to the State Enrollment form and forward to Risk Management Division.
- Group Representatives should not begin deductions for anyone who was required to submit Evidence of insurability until approval has been received from The Standard. In addition, payroll should be notified as soon as possible to have deductions taken out for the pay period that benefits go into effect.
