New hires of State Agencies and Local Public Bodies are eligible for Additional Life coverage up to three times annual earnings without evidence of insurability if applying within 31 days of becoming eligible. Coverage requests in excess of the Guarantee Issue amount of three times annual earnings are subject to medical underwriting approval.
Legislators are eligible to elect Additional Life coverage up to $150,000 without evidence of insurability if applying within 31 days of becoming eligible. Coverage requests in excess of the Guarantee Issue amount of $150,000 are subject to medical underwriting approval.

Evidence of insurability is required for late applications for Life and Dependents Life insurance. Please refer to the Group Life Insurance Certificate for complete details. Employees and dependents are considered late enrollees if they meet the plan rules for eligibility but do not enroll within 31 days of becoming eligible.
To apply for medical underwriting approval, first complete the State Enrollment/Change Form with your request to add or increase Additional Life and submit to your Human Resources Representative. Next, complete the Medical History Statement (access here) or online at the New Mexico General Services Department website, forms section. Please complete all items, date, and sign as instructed.

Please allow six to eight weeks for full process time of your Medical History Statement. A physical exam and/or physician statement may be required. The Standard will notify you of any requests and decisions via the mailing address or phone number you provide.
