The State of Nevada provides a basic amount of Life insurance to help protect your loved ones in the event of your death. Since everyone's needs are different, the State of Nevada also provides you with the opportunity to apply for Voluntary Life insurance from Standard Insurance Company — a simple, easy way to further help protect your family. It allows you to apply for the additional coverage you need, with premiums deducted directly from your PERS check in most instances for Retired Participants. In certain circumstances, you may be required to provide satisfactory proof of evidence of insurability.
If you participate in the State of Nevada's PEBP–sponsored medical plan, the State provides Basic Life and AD&D insurance. The Basic plan is $10,000 of Life insurance and $10,000 of AD&D coverage. The State also provides Dependents Life insurance in the amount of $1,000 for your spouse and eligible children.

You may also purchase Voluntary Life insurance at group rates. Voluntary Life amounts may be elected in a multiple of $5,000 to a maximum of $50,000 with a minimum of $10,000 of coverage.
Effective July 1, 2006, Voluntary Life premiums for enrolled PEBP retired participants are calculated based on the participant's age as of each July 1. This means that regardless of the amount of insurance elected, retired participants will pay premiums based on their age and the amount of coverage elected. A composite monthly premium for the first $10,000 of insurance coverage will no longer be offered.

As a reminder, Standard Insurance Company maintains all Life insurance beneficiary designations, including designations for Basic Life and AD&D insurance. If you want to designate or update your beneficiary(ies), please complete the Beneficiary Designation and Change Form and return it to the address listed above.
