To enroll or make changes using the online Enrollment Tool, follow these instructions and then enroll at http://www.standard.com/enroll. For paper enrollment simply complete the enrollment form, print, sign and mail it by May 31, 2012 to:

Important Notes

To enroll in The Standard's Voluntary STD plan you cannot be currently enrolled in another STD plan, such as Colonial or Aflac. If you are, simply contact your Agency Representative or PayCenter (e.g., Central Payroll, NDOT) and request they stop that payroll deduction.
