
You have two options for coverage continuation if your insurance with Maricopa County ends: Conversion and Portability.
If your Life insurance ends or is reduced (for any reason other than failure to make a required premium contribution or payment of an Accelerated Benefit), you may convert qualifying amounts of your coverage to an individual life insurance policy without providing Evidence of Insurability.
You must apply for Conversion in writing and pay the first premium within 45 days after termination of, or reduction in, your coverage. Premium rates for converted coverage differ from rates for coverage under the Group Policy.
Dependents may also convert qualifying amounts if their Life insurance ends. AD&D Insurance may not be converted.
If your insurance ends because you cease to be a Member due to a reduction in your scheduled work hours, termination of your employment or an unpaid leave of absence, you may continue ("port") qualifying amounts of your group Life, AD&D and Dependents Life coverages.
You must elect Portability in writing and pay the first premium within 45 days after you cease to be a Member.
Additionally, in the case of your death or divorce, your Spouse may apply for portable Dependents Life insurance for him or herself and any eligible Child(ren).
Please note that if you do not port your Life insurance, you may not port AD&D or Dependents Life insurance. Your Spouse must elect to port Dependents Life insurance for him or herself in order to port Dependents Life insurance for your Child(ren).
Premium rates for ported group coverage differ from rates for coverage under the Group Policy.
