Maricopa County provides a Basic Life and AD&D insurance plan at no cost if you are an active regular employee scheduled to work at least 20 hours per week, an elected official, or benefit-eligible contract employee. The Basic plan is 1 times your annual earnings to a maximum of $500,000, for both Life insurance and AD&D coverage.

Additional Life and Dependents Life Insurance

The County recognizes that individuals have different needs and has provided you the opportunity to apply for the right amount of protection for you and your family at very competitive group rates. Additional Life may be elected in the amount of 1 to 5 times your annual earnings to a maximum of $1,000,000 when combined with Basic Life. Dependents Life for your spouse may be elected in any multiple of $10,000, to a maximum of $100,000. Dependents Life for your child(ren) may be elected in any multiple of $5,000, to a maximum of $20,000.

Voluntary AD&D Insurance

You may choose to elect Voluntary AD&D insurance in the amount of 1 to 5 times your annual earnings to a maximum of $500,000, to cover you or both you and your dependents.

Some late enrollments and requests for coverage increases are subject to medical underwriting approval.
