Hennepin County offers Basic Life and AD&D insurance at no cost, and you don’t need to fill out an enrollment form. To enroll in for Additional Life and AD&D insurance, please call Human Resources – Benefits Unit at 612.348.3530.
To enroll for STD insurance complete the enrollment form, print it and return the completed form to Benefits, A700 Government Center, Mail Code 070. Be sure to keep a copy of your completed form for your records.
Remember, you are automatically enrolled in LTD insurance on the first day of the pay period following 30 days as an eligible employee, unless you waive your participation. To waive coverage, you must complete a waiver card. The waiver card is available at New Employee Orientation or from the Human Resources Department (Benefits Unit). The waiver card is to be completed only if you do not want to participate in the LTD coverage.
Note: If you are required to provide Evidence of Insurability, please complete a Medical History Statement. Coverage subject to Evidence of Insurability is not effective until approved by The Standard.
