Government of the District of Columbia provides you with the opportunity to purchase Basic Life and Basic AD&D insurance. Basic Life and AD&D insurance become effective immediately, provided you are actively at work on that date. Basic Life insurance is equal to one times your Annual Earnings plus $2,000 and the amount of AD&D coverage is equal to the amount of life coverage. Basic Life has a minimum of $10,000.

Government of the District of Columbia, recognizing that individuals have different needs, and has provided you the opportunity to apply for additional protection for you and your family at very competitive group rates. You may elect Additional Life coverage Option A: a flat $10,000 benefit, Option B: any multiple of one, two, three, four, or five times your annual earnings, or Option C: Dependent Life insurance in amounts of $5,000 for your spouse and $2,500 for each child, plus Option A or B.

In addition, you may qualify for an Extra Benefit amount. This provides an extra percentage of the amount of your Basic Life insurance benefit, from 10% to 100% depending on your age at the time of death. The Extra Benefit is payable only if you die before age 45.

In certain circumstances, you may be required to provide evidence of insurability.
