Basic Life and Accidental Death and Dismemberment (AD&D) Insurance

The Standard is The CSU's insurance carrier for employer-paid Basic Life and AD&D insurance.

To be eligible for employer-paid Basic Life and AD&D insurance you must:

• Be a Member
• Complete your Eligibility Waiting Period, and
• Meet the requirements in Life Insurance and Active Work Requirements

Definition of Member:

You are a Member if you are an active employee of the Employer, and one of the following:

1. Appointed half-time or more for more than six months in an executive, management, confidential, bargaining Unit 1, Unit 3, teaching associate, Unit 4, Unit 8, CSUEU (Units 2, 5, 7, 9) or Unit 12 position; or
2. Appointed for at least six (6) weighted teaching units or more for at least one semester or two or more consecutive quarter terms in a lecturer or coach academic year position (Unit 3)

You are not a Member if you are a full-time member of the armed forces of any country.

Voluntary Life

To be eligible for Voluntary Life insurance you must:

• Be a Member
• Complete your Eligibility Waiting Period, and
• Meet the requirements in Life Insurance and Active Work Provisions

Definition of Member:

You are a Member if you are one of the following:

1. An active employee of the Employer appointed half-time or more for more than six months in an eligible employee category; or
2. An active employee of the Employer in certain academic year classifications may be eligible if appointed for at least six (6) weighted teaching units or more for at least one semester or two or more consecutive quarter terms.

You are not a Member if you are:

1. A full-time member of the armed forces of any country; or
2. Insured under CIGNA group universal life insurance policy number 03-M101220

Long Term Disability Insurance

The Standard is The CSU's insurance carrier for employer-paid Long Term Disability (LTD) insurance.

To be eligible for Employer paid LTD insurance you must:

• Be a Member
• Complete your Eligibility Waiting Period; and
• Meet the requirements in Active Work Provisions and When Your Insurance Becomes Effective

Definition of Member:

You are a Member if you are a) an active employee of the Employer, b) a citizen or resident of the United States or Canada, and c) one of the following:

1. Appointed half-time or more for more than six months in an executive, management, supervisory, bargaining Unit 4, bargaining Unit 3 (excluding Faculty Early Retirement participants), or bargaining Unit 1 position; or
2. Appointed for at least six (6) weighted teaching units or more for at least one semester or two or more consecutive quarter terms in a lecturer or coach academic year position (Unit 3)

You are not a Member if you are:

1. A temporary or seasonal employee; or
2. A full-time member of the armed forces of any country
