How to Enroll During the 2010 Open Enrollment Period

To elect or change coverage, please access the Enrollment and Change Form and complete Sections A, B and C online, then print, sign and return it to your department's Personnel Office no later than April 30, 2010 for processing. You may want to keep a copy for your files.

Before enrolling, review the Voluntary Long Term Disability Insurance Booklet and Certificate of Insurance for answers to commonly asked questions, costs, exclusions, limitations and reductions.
