The State of Arizona provides a Basic Life and AD&D insurance plan at no cost if you are an active permanent employee who regularly works at least 20 hours each week. The Basic Life and AD&D insurance becomes effective when you are hired, provided you meet the active work requirement on that date. The Basic plan is $15,000 of Life insurance and $15,000 of AD&D coverage.
The State recognizes that individuals have different needs and has provided you the opportunity to apply for the right amount of protection for you and your family at very competitive group rates. Additional Life and AD&D amounts may be elected in any multiple of $5,000, not to exceed the lesser of three times your annual earnings, to a maximum of $300,000. Dependents Life and AD&D coverage amounts vary from $2,000 to $15,000. The Life and AD&D insurance coverage includes many Features and Provisions.

Long Term Disability (LTD) insurance is designed to pay a monthly benefit to you in the event you cannot work for 6 months or longer because of a covered illness or injury. This LTD benefit replaces a portion of your income, helping you to meet your financial commitments in a time of need.
As a benefits-eligible employee, you are automatically enrolled in one of the State of Arizona's two Long Term Disability (LTD) programs, starting with your first day of work. The retirement system to which you contribute determines the LTD program available to you.

If your LTD claim is approved by The Standard, LTD benefits become payable at the end of the 180-day benefit waiting period. No benefits are payable during the benefit waiting period. The monthly LTD benefit is based on your earnings from the State of Arizona. Your monthly LTD benefit is 66 2/3% of the first $15,000 of your monthly earnings, as defined in the group insurance policy, reduced by Deductible Income. The maximum LTD benefit is $10,000 per month.
