This booklet is designed to answer some common questions about the group Long Term Disability (LTD) insurance provided to eligible participants of the State of Arizona who participate in certain State of Arizona retirement programs. The controlling provisions will be in the group policy. This booklet does not modify that document or the insurance in any way.
