Copies and Faxes Accepted for Certain Documents and Requests
You asked, and we listened. In order to provide better service the annuities administration team has made a few changes to our policy regarding the need for original paperwork. Photocopies, faxes, or e-mailed PDF scans for certain documents or requests will now be accepted.
To better meet the needs of beneficiaries and policyowners on death claims and distribution requests, photocopies of death certificates and our Substitute W4-P and W-9 form (5031) will now be accepted. This would also include the IRS W-9 and W4-P forms. In addition, copies of EFT forms and voided checks are accepted. Note, that the term “copies” means that we will also accept a fax in these instances.
Also, to speed up the process for certain distribution requests, the paperwork for partial distributions of $10,000 or less that will be sent to the address of record can be faxed to the team. In order to accept a photocopy or faxed copy of this type of distribution request there can be no special handling such as a POA or sending the check to another address.
Administration requests not mentioned above, or special handling situations, require original documentation and signatures in order to enact it.
